Inwards. PVI designs the standard threshold ramp for doorways that swing in, so it sits outside the door's path. For a door that swings out, look at the ELEV8, which butts right up against the door.

Either. It has holes punched in the top corners and PVI supplies the screws to fix it to the floor, but you don't have to mount it permanently.
